Our doctor reviews your heart health, breathing capacity, and general fitness to make sure your body can handle the extreme stress of the race. By catching risks like an undiagnosed heart issue or severe high blood pressure before you start, this medical acts as a crucial safety net, helping to prevent medical events on race day and confirming your fitness-to-run certificate meets the organiser’s official rules

Which endurance events typically require this medical certificate?

This medical certificate is commonly mandatory for major international events, including most European marathons (like Paris, Rome), large ultra-marathons, challenging cycling events (such as L’Etape du Tour), and certain long-distance triathlons. Always check the official entry requirements of your specific race organiser.

The certificate’s validity is set by the race organizer but is typically valid for one year from the date the doctor signs it. You must ensure the certificate is still valid on the day of your race.

We recommend booking your medical 1–2 months before your race day. This provides enough time to get the official certificate, and crucially, allows you time to seek further specialist review (e.g., a cardiologist) if the doctor identifies a potential issue during the assessment.

Law in France and Italy requires all competitive athletes to prove they have no “contraindications” to sports. Without this stamped certificate, you will not be allowed to collect your race bib.
While some basic certificates only require a physical, many events (and British Sports Medicine guidelines) now strongly recommend a Resting ECG to screen for underlying heart conditions like HCM or arrhythmias.
The form must usually state that there are “no contraindications to competitive running.” We are familiar with the specific templates for the Paris, Rome, and Florence marathons.
A high BMI isn’t an automatic fail, but our doctor will assess your overall cardiovascular health. If your blood pressure and heart rate are within safe limits, we can usually clear you for the race.
If an abnormality is found, we may not be able to sign the form immediately. We might refer you for a private echocardiogram (heart ultrasound) to ensure it is safe for you to compete.
