Our GPs are experienced in completing AH1 (Adult Health Report) Form AH2 (Foster Medical Review) or Medical Advisor forms. We can also give the panel indepedent Mental health opinions as we have a Psychiatrist with experience in the field on our team.

Fostering New Applicant Form (AH1)

For the initital medical for new applicants (AH1) – our medical secretary can obtain full medical records from GP if required or you can use a medical summary – 48 hour turnaround if required- fee for this service is £125

Fostering Form (Adult Health Report) AH2 

The AH2 form is an update or review medical form to be completed if there are health changes or every three years. 

Our GPs are experienced in conducting the medicals, we have contracts with local authorities and foster medical agencies nationwide. We would be happy to support you as a private client with your medical or your organisation if you require our services.

What is a Foster Medical

A Foster Carer Medical is a mandatory health assessment required by all local authorities and independent fostering agencies for prospective or renewing foster parents. The core purpose of this medical is to ensure that your current and past physical and mental health status is stable enough to allow you to provide a safe, consistent, and reliable care environment for vulnerable children.

What is the main purpose of the medical?

The medical assures the Adoption Panel that you are physically and mentally robust enough to meet the complex needs of a child throughout their dependency, focusing on your long-term health and parenting capacity.

The medical covers factors that impact long-term health, including BMI, smoking/vaping status, and alcohol consumption. You may be asked to stop smoking for a period (often 6 months) before being approved to adopt a young child.

The appointment usually takes 30–60 minutes, as the doctor needs time to review your records, conduct the examination, and complete the detailed CoramBAAF form.

This is the industry-standard form (Form AH for Adoption or Form PH for Fostering) used by agencies to assess if you have the “health and vitality” to raise a child into adulthood.
